Original Official Receipt assessment fee
NSO/PSA Birth Certificate (Photocopy)
2pcs. Passport size picture (White background any dark T-shirt with collar no name tag)
Certificate of Employment/Endorsement Letter sign by the Authorized personnel
– 1 copy of photocopy of Marriage Contract, if married.

– After filing, when will I take my assessment?
Answer: The Assessment Center Office will give you an update regarding your schedule since we have to fill ten (10) persons in one batch. Also, it depends the approval from TESDA office because they will check the availability of TESDA representative.

– Did you accept partial payment for assessment fee?
Answer: No.

1. 1 copy of Photocopy Birth Certificate (any available PSA/NSO/Civil Reg. Copy)
2. 3 pcs Passport size with collar and white back ground
3. 2 pcs ID picture
4. 1 copy of Photocopy of any available (transcript of record/form 137/form 138 card/evaluation copy of grades)
5. 1 copy of photocopy of Marriage Contract, if married.
